Use this form to Order your prescription from us - please note - for collection of prescriptions from our Pharmacy in Palmerstown Dublin 20, Ireland - We do not post or deliver prescription medication.

Use this form to help us have your prescription ready for you to collect , we dispense private and HSE scheme prescriptions, including General Medical Services (GMS), Drug Refund Service (DPS) and Long Term Illness (LTI)

If possible upload an image of your prescription. Please note original prescription must be presented in our pharmacy or if its an electronic prescription it can be sent be health-mail to
